Certificate of Immunization

Colorado state law requires all students born on or after January 1, 1957, to provide proof of immunization for measles, mumps and rubella (MMR), Prior to your arrival on campus, please send any formal immunization record – or a completed immunization form signed by a doctor, nurse or school official – to Student Affairs Office.
